Sunset’s Life Plan is foundational to how we approach our mission to serve. This plan is an ongoing assessment of a resident’s current needs and matching those needs to our specific services. Sunset’s Life Plan is adaptable to each resident and ensures a quality of life that nurtures the physical, mental, and spiritual well-being of those we serve.
Much like life itself, this continuum of care does not follow a rigid course, but adapts to changing needs and circumstances. It is a proactive opportunity for us to stay actively engaged with our residents and enables us to serve efficiently and effectively within the variety of services we offer.
There are three key elements of our Life Plan:
- Understanding residents’ basic human needs that lead to well-being
- Ongoing assessment of residents’ status in these needs
- Matching our services to specific needs
